Forms To Complete For Your Sleep Study Visit
In preparation for your sleep study at the sleep center, which is located in the Fairfax office, print the appropriate Sleep Forms packet, complete all the forms and bring the completed forms with you.

If you are a current patient of Dr. Cho’s, or another physician at the Neurology Center of Fairfax, print and complete the Current Patient Forms packet.

If you are not currently a patient at the Neurology Center of Fairfax, print and complete the New Patient Sleep Forms packet.

If you are coming in for a follow up to your sleep study appointment, please fill out the Follow-Up Appointment Forms packet.

Insurance

Obtaining Prior Insurance Authorization
You MUST have prior authorization from your insurance carrier for your sleep study. To obtain the required insurance authorization and to schedule a sleep study at the Sleep Diagnostic & Treatment Center laboratory, please print the Sleep Study Guideline Form. You must complete this form and return it to us via fax, 571-308-1158, so we can work to obtain prior authorization from your insurance company for your sleep study. Your sleep study cannot be scheduled until this insurance approval is obtained (unless you are a self-pay patient, paying cash for your treatment).  This process may take up to two weeks to complete.
